1.     Application Process

The first step in the admission process is the application.  Secure, complete, and return the student application forms and information to the appropriate school office.

Once the above items have been completed, the family will be contacted to schedule an interview.  The student will need to be present for the interview.  After the interview, a testing date will be scheduled for the applicant, if appropriate.

If applicable, a Financial Aid Application may be obtained from the school office or may be completed online at www.ffna1.com.  This application is submitted directly to Family Financial Needs Assessment.  Please, do not submit to the school office.

2.     Interview Process

The following documents must be brought with you for your interview:

Copy of Birth Certificate
Copy of custody papers (if applicable)
Original blue Florida Certification of Immunization Form (DH680)
Original yellow State of Florida School Entry Health Exam Form (DH3040)
Copy of Psychological Testing (if applicable)
Request for Student Records Form (Grades 1-12)
Copy of School Recommendation Form (Grades 1-8, highly recommended)

3.     Acceptance and Registration Process

After the interview, any necessary testing, and upon the Principal's direction, please be prepared to complete the following documents and submit all applicable fees in the school office:

Financial Agreement Form
     FACTS Form and voided check (if paying monthly over 10 months)
     $41 FACTS Fee
$200 Registration Fee
Safety Form
Internet Agreement Form (Grades 1-12)
