Athletic Participation Forms

All student-athletes must submit FHSAA paperwork each school year in order to be eligible to participate in athletics at FBA. Forms are valid for 365 days.

A sport participation fee of $100/$125 (football) per sport/per season, will be invoice via your RenWeb account.

What ImPACT Testing?

FBA uses Immediate Postconcussion Assessment and Cognitive Testing (ImPACT), a computer-based evaluation for sports concussions.  A pre-concussion ImPACT test helps establish a baseline cognitive functioning.  This baseline can be compared to post-concussion testing if the athlete suffers a head injury later. 

Why should athletes get baseline testing?

Comparing the pre- and post-concussion test scores can help determine when an athlete is ready to return to normal activities (athletes may not return to play until all concussions symptoms have subsided).  ImPACT is one of many tools our doctors use for athletes with concussions.  Before the season starts, an athlete can take the test to measure attention, visual memory, reaction time and more.  This brief series of tests take about 30-45 minutes to complete.  Parents receive a copy of the results.

When is the ImPACT offered?

The ImPACT is offered prior to the beginning of each season for any athlete that has not received a baseline.  Athletes only have to take the assessment once in the two years they are here at FBA.  There is no cost for the ImPACT test, and the test is administered by our Athletic Trainer.

Over the summer several student-athletes plan to participate in FBA team workouts/practices, open gyms, camps, etc... In order to do so, you must be re enrolled as either a full time "traditional" student or a Home School Extension (HSE) Student at FBA. If you have not yet re enrolled and plan to be a part of some summer athletic team activity at FBA, please do so before the first start date of your summer sport participation. All of the required sports forms must be on file with the Athletic Office prior to participation; this includes joining summer athletic events.  Just to reiterate, no student-athlete will be permitted to participate without the proper paperwork on file. All forms must be current. If you are unsure when your student-athletes athletic forms expire, please contact Cindy Murphy.

If you do not plan to participate in a summer activity but plan to join a team during the 2017-18 school year, you must have all of your sports paperwork in to the Athletic Office before the first official practice day of your sport.
